On-Site Available Healthcare Assistance at Heritage Oaks

Healthcare Assistance is available through AT Home Care, a healthcare provider that we have worked with for 11 years but is independent of Heritage Oaks. The personal care program features eight different service options. Residents can choose one or several services and will incur a higher cost as the number of services increase. Health care assistance enables residents to “age in place” and is a wonderful, affordable alternative to many assisted living facilities.

AT Home Care also provides a Medication Management Program that includes medication set up, medication delivery, a meal program, monthly weight and vitals check-ups and a monthly RN visit for supervision.

• Dressing and grooming assistance
• Housekeeping services (light cleaning of living area, bathroom and kitchen)
• Personal laundry service
• Meal program (includes meal delivery or taking resident to dining area)
• Bedtime services (basic ADL assist in relation to bedtime preparation)
• Nightly check-in
